Always a "when" these days, the times when it was an "if" are long gone.

In general the root cause can be traced back to decision makers deciding that preventive security was too expensive, would take too long to implement or even that it was someone else's problem so they could ignore it completely.

Still, it's only the customer what suffers, innit?
